Support Coordination Update
Mercy Community has made the difficult decision to stop providing Support Coordination and Specialist Support Coordination services. We are supporting participants through this change, and working to progressively support transitions to new service providers. Mercy’s Support Coordination services will cease from 31 January 2025. Mercy Community is not accepting new referrals for Support Coordination or Specialist Support Coordination services.
