Achieve greater independence to self-direct
Connect to mainstream supports
We take a holistic approach to delivering two levels of Support Coordination under the NDIS:
Support Coordination:
This is a capacity building support that helps optimise the way you use supports in your plan in the following ways:
Help you work towards your goals through greater understanding of your NDIS plan
Links you to the government services, community, mainstream and NDIS providers
Increase your skills and confidence when coordinating your supports
Assist you to manage your NDIS budget
Specialist Support Coordination:
Higher intensity Support Coordination. We help you action your NDIS plan and reach your goals when you and your support network partner with us.
If you have been funded for Support Coordination you’ll find it under your “Capacity Building” support category of your NDIS plan.
How much does it cost?
Mercy Community prices are in line with the NDIS Price Guide while following the NDIS Code of Conduct and Practice Standards.

Mercy Community and the NDIS
Mercy Community has been supporting people with a disability for almost 130 years. We are a registered National Disability Insurance Scheme (NDIS) provider, dedicated to helping you make the most of all that is available to you through the NDIS.
We have a dynamic team of support workers who, every day, assist people with a disability throughout Queensland to live as independently as possible.
