Mercy Community’s Connected Families Intensive Family Support Program is designed to empower families with children and/or young people up to 18 years of age on their journey toward positive change. Our mission is to work collaboratively with you, offering support and guidance to manage and improve various aspects of your family life.
Explore our individual and family support programs today. Families can self-refer or be referred by others with the consent of the family (certain state government services can refer without consent).

Eligibility for Support
Our program supports families with children ranging from unborn infants to 18 years of age, experiencing multiple and/or complex needs, and would benefit from intensive and specialist services, you qualify.
Additionally, if lack of support could potentially involve a child in the child protection system, you are eligible for our assistance.
